The parent or legal guardian of a student coming to a Chicago Public School for the first time needs to present to the school office proof of age of the student, proof of current address. Proof of a physical, dental, and eye exams are required before the start of the school year.

Students enrolling in K, 2nd, and 6th grade must also have a dental exam.
